How to Apply

Submit a Completed Application Your completed application must be postmarked or received by December 23, 2011.

An application will not be considered unless it is complete. A completed application includes:

• Two-page application form

• Student Essay answering 3 questions

• Copy of current and past year student report card (Transcripts are required for 10th and 11th grade transfer students).

• 7th grade ISAT scores

• Two letters of Recommendation from:

1. A personal mentor, school counselor, pastor, coach, etc AND

2. An academic teacher

Decision Timeline

• March 2012: Family Information Meetings for prospective students (by invitation only)

• April 2012: Decision letters mailed

• Summer 2012: Waitlist admissions and 10th and 11th grade openings filled.

Open House Dates:

Saturday, November 5, 2011 ~ 10 am-12 noon
Saturday, November 19, 2011 ~ 10 am- 12 noon

Admission Policy

Collins Academy High School is a non-selective enrollment school of choice that is open to students from all across Chicago.

The application process for Collins Academy is intended to assist students and their families in making an educated school selection and to ensure a good match for the student and family.

Parents and students are encouraged to attend one of the Open Houses to learn more about the school.

Transfer Applications

Applications for 10th and 11th grades will be accepted at any time. However, it is unknown how many openings there will be (if any) until late summer. Decisions are made as openings become available. We do not accept applicants for the 12th grade.
